Our Client’s Problem
Upham’s Community Care pinpointed a significant gap in healthcare accessibility exacerbated by the COVID-19 pandemic. Residents struggled with transportation barriers, scarce healthcare provider availability, and economic hardships, further widening the healthcare access divide.
Our Approach
STEP 1: Secured Funding.
Day Health worked with Upham’s Community Care to apply for the COVID-19 Community Health Equity grant from the Boston Public Health Commission. This collaboration to obtain funding focused on expanding Upham’s capacity to deliver healthcare services directly to community members, particularly those affected by delays in care due to the pandemic. The grant application focused on strengthening Upham's ability to reach underserved populations. By doing so, the project aimed to dismantle barriers to healthcare access and foster a healthier, more connected community.
STEP 2: Designed and Delivered.
Once the grant was awarded, Day Health Strategies developed a detailed design for the Mobile Health Initiative, focusing on the most feasible and cost-effective mobile health services. Our design was anchored on three pillars:
Comprehensive Needs Assessment:
We conducted a series of leadership, staff, and patient interviews.
Through a thorough analysis of the interview data, we identified a strong demand for a range of services including blood pressure checks, eye care, A1C/glucose testing, dental care, STI/HIV/pregnancy testing, vaccines, rapid flu/strep tests, and health navigation.
This data-driven approach ensured our services were aligned with the community's most pressing health needs.
Service Design and Implementation:
Utilizing a prioritization matrix, we outlined a 3-phased rollout plan beginning with general health education and screening services (Phase 1). Phase 2 included vaccinations and social determinants of health (SDOH) screenings, and Phase 3 culminated in a comprehensive suite of testing and preventive services.
Each phase was meticulously designed to include essential staffing, space, supplies, and communication strategies, tailored to maximize community impact and ensure sustainable implementation.
Key services slated for the initial phases included blood pressure checks, A1C checks, and dental screenings, with an emphasis on accessibility during nights, weekends, and off-hours to reach the widest possible audience.
Compliance and Evaluation Framework:
Recognizing the importance of adherence to regulatory standards, our plan included obtaining necessary DPH and HRSA licenses to legally and ethically provide mobile services.
We then established a robust monitoring and evaluation framework to measure the initiative's effectiveness in advancing specific goals such as:
improving equitable access to preventive services,
integrating services across providers,
strengthening community partnerships,
enhancing the initiative's brand visibility, and
generating sustainable revenue.
